Formal written license verifications by the Board of Examiners of Psychologists can be provided upon request.  The request must be made in writing and include the following information:

Ø  Name of the licensee,

Ø  Address where the completed verification is to be mailed; and

Ø  A money order or check in the amount $20.00 made payable to the Board of Examiners of Psychologists.
