Requirements for Submitting Written Testimony and Comments

 
Individuals, corporations or other organizations may submit written testimony and comments to be considered by the Maryland Department of Health and Mental Hygiene (DHMH) Pharmacy and Therapeutics (P&T) Committee.  To ensure sufficient time for review, the material must be received at DHMH at least one week prior to the relevant meeting.  This material may be submitted via e-mail, U.S. Postal Service, or commercial delivery service.  E-mail is preferred; however, it must be compatible with MS-Word or in Adobe® Acrobat format.  Charts and graphs may be submitted in MS-Excel or Adobe® Acrobat.   

E-mail your testimony or comments to MarylandPDLQuestions@dhmh.state.md.us.  Mail or deliver 13 copies of your material to The Pharmacy and Therapeutics Committee, Department of Health and Mental Hygiene, Suite 408F, 201 W. Preston Street, Baltimore, Maryland 21201.  Your testimony will be forwarded to the P&T Committee members if it is submitted via e-mail or is packaged in individual envelopes with postage attached to 12 of them.  Binders and heavy material must be placed in individual padded envelopes. 

 
Please do not expect a response other than an acknowledgement that your testimony was received.
