  What Is Medical Day Care?

Medical Day Care is a structured group program that provides health, social, and related support services to functionally disabled adults, age 16 and older. The Program provides individuals the opportunity to receive medical care during the day in a community-based setting offering individuals an alternative to nursing facility care.

  How To Apply For Services?

Individuals can access medical day care services through one of the following home and community-based waivers: Medical Day Care Services Waiver, Older Adults Waiver, Living At Home Waiver, Traumatic Brain Injury Waiver, Model Waiver, Community Pathways Waiver and New Directions Waiver. Individuals may be enrolled in only one home and community- based waiver at a time.

If you are interested in receiving medical day care services through a home and community-based waiver, please contact the local health department AERS (Adult Evaluation and Review Services) Program in the area in which you reside.
